A medical questionnaire is required to qualify.
Prism
PrecisionTM provides basic coverage and is
a practical alternative for the services and treatments not
covered by the government. This coverage is guaranteed and no
medical questionnaire is required.
Prism
ContinuumTM focuses on
providing coverage for those leaving a company group health
plan. Coverage is guaranteed if you apply within 60 days of
losing your benefits.
